Aged Care Quality Standard 2
Standard 2 stresses the importance of continuous partnership in care planning, ensuring services remain attuned to changing health, goals, and circumstances. It supports clients in staying engaged in their well-being, reinforcing trust and improving life quality.
Under Standard 2, providers must:
• Provide ongoing partnership in assessment and planning.
• Document care plans clearly, reflecting client goals and risk management.
• Engage all relevant parties, including external providers, with the client’s consent.
• Ensure plans are up-to-date and accessible at the point of care.
• Respond promptly to changes or incidents with plan revisions.
• Clients benefit from personalized, evolving care that respects their preferences.
• Staff gain clearer direction and coordination, reducing risk and inefficiency.
• Providers improve compliance, reduce incidents, and foster continuous quality improvement.
A failure to meet Standard 2 risks serious consequences. In January 2025, 16 Australian aged care services were found non-compliant, with one example involving a resident continuing to eat bread despite choking risks—part of inadequate assessment and planning processes.
